Transportation Waiver
Parents or Guardians that prefer to provide for the transportation of their child to and from athletic events independent of school-provided transportation must fill out and sign both pages of a copy of the Regional School District #12 Waiver of Transportation, and their child must present the signed form to the coach before the team departs for the athletic event. When a parent follows this procedure, parents will be authorized to provide transportation for their children independently of the the school.
Under no circumstances will students be permitted to drive themselves to or from athletic events.
Both sides of this form must be completed and submitted by hand, and electronic/digital copies cannot be accepted.
